West River Health Services (WRHS) is excited to announce that we have recently partnered with Caravan Health in lowering the cost of healthcare to our Medicare beneficiaries.
WRHS will be participating in improvement activities based around Annual Wellness Visits and other proactive healthcare visits.
The healthcare system can be confusing to navigate at times, so to assist our patients in their healthcare needs, we have added the position of Population Health Nurse to our staff. This position will be tasked with coordinating care between WRHS and specialists when required, reminding patients of upcoming appointments, and periodic follow up calls to determine if there is anything else that we can be doing for your health.
