As of May 1, we have initiated a new Electronic Health Record (EHR) called Epic One Chart. As part of this, we now participate in a portal called My Chart. This portal allows you to view your medical records and lab reports. You can also communicate with your healthcare team. If you already have access to My Chart through another facility, it will operate with West River. If not, we encourage you to contact your local WRHS clinic to sign up for My Chart. You can also sign up as a proxy for your family members.

Follow My Health will be deactivated from our system, on May 27 at the close of business. We encourage you to sign up for My Chart before that. We hope this will be a smooth transition for you and allow you even better access to your chart and your care team. We appreciate your support of our healthcare system as we continue to provide healthcare right where you need us.
