Western Horizons Assisted Living is a partner of Western Horizons Living Centers which include Western Horizons Care Center. This organization and the services provided along with the hospital and clinic services offered by West River Health Services provide a complete rural continuum of care for our region.

Continuum of care means that we provide a team based model of care led by the tenant’s personal physician and healthcare team who provide continuous and coordinated care throughout their lifetime. The clinic and hospital are conveniently located to maximize health outcomes. The continuum of care is here, in our community, among family and friends.

Assistance: Service Payments for the Elderly and Disabled (SPED). SPED is state funding for the personal care services offered at an assisted living facility, for those who meet the financial and functional eligibility. The asset limit is $50,000 and there is a sliding fee scale used for monthly income.
